Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

 

• Do I have mental health benefits?

• Will those benefits pay for work with a Licensed Professional Counselor?

• What is my deductible and has it been met?

• How much is my co-pay per session?

• How many sessions per calendar year does my plan cover?

• Is approval required from my primary care physician?

 

Additional Fees: 
Additional fees may apply in the form of a co-pay if cost of service is not fully covered by your insurance carrier. There is a $30 returned check fee.
